This thread is resolved. Here is a description of the problem and solution.
Problem:
The issue here is that when the user creates a form they are redirected to a page that only has the form on it. The form seems to be a relationship form.
Solution:
This issue has been resolved in our latest 2.1 version.

Instead, I got: A blank page as per the screenshot. Notice the url, that is the plugins page. I have toolset and all the rest updated to latest version.
Can this behaviour be replicated on your side?
