Is there any way that I can assign the same form, to different post types? either from backend or frontend, programmatically?

To debug this issue you should:
- Make sure that you are running with latest PHP and WordPress version
- Make sure that you are using latest Toolset plugins
- Check for any possible Javascript errors on browser console

With above image you can see there is a button called "Advanced Options" when you will click on it

it will show you section "Spinners" from where you can setup spinner image.
Note:
Please make sure that when you click on "Advanced option" button under section "Cache and preload" section option with checkbox "Pre-load the next and previous pages - avoids loading delays when users move between pages" should be unchecked.
