I am trying to: place the post form short code to a page, the radio button to insert the new post form is disable. so i use edit post form radio button to insert my contact form.

I can see there is a message "post type mismatch" in your screenshot:

That means you put the CRED form link into wrong place, I assume you are going to use CRED form to create new custom posts, in your first screenshot:

You will need to choose option "New Post Form", in option "Select post form" choose the specific form name.

There isn't such a built-in feature within CRED form, it needs custom codes, I have seen other users use Thinkbox to build such kind of popup CRED form, see below thread:

Which view shortcode are we talking about? Where do you want it display?
If you just need to display the Views shortcode [wpv-view] in the widget, you can simply create a text widget, and copy/paste the Views shortcode [wpv-view] into widget content

No, Views works like this, you put a [wpv-view] shortcode into a page, with the URL of page, then you can setup the HTML link.
